Description of problem:

The mod_form package contains
/usr/include/httpd/mod_form.h

The mod_form-devel package contains
/usr/lib64/httpd/modules/mod_form.so

It should be the opposite way around.

Version-Release number of selected component (if applicable):

$ rpm -qpl mod_form-0.1-2.20131204svn145.el7.x86_64.rpm 
/usr/include/httpd/mod_form.h
$ rpm -qpl mod_form-devel-0.1-2.20131204svn145.el7.x86_64.rpm 
/usr/lib64/httpd/modules/mod_form.so

The spec file is wrong here:

This module does not seem to be used frequently.

Workaround: install mod_form-devel

Also broken in epel-6 and Fedora.
